This is a simple giveaway with a form to fill out, open to all of my followers. Just fill in your responses to the form at the bottom of this post and you're all set! The last day to enter the giveaway is December 20, 2012.

The prize for this giveaway will include Konad nail art stamping plates, which were generously provided by my friend at Wow Wow Nails. Wow Wow Nails is where I get all of my Konad supplies and it is through Wow Wow Nails that I started getting into nail art and began blogging.
The prize will also include nail polish, of course. I will be going prize polish shopping soon so I will let you guys know what I end up picking for the winner.
